Consolidated Health Care Centers Program

Program Description
The Consolidated Health Care Centers Program is a federal benefit program providing comprehensive primary and preventive health care and social services to medically unserved and underserved populations. Federally funded health centers are community-based and patient-directed organizations that serve populations with limited access to health care. These include low income populations, the uninsured, those with limited English proficiency, migrant and seasonal farmworkers, individuals and families experiencing homelessness, and those living in public housing. Services are provided to all residents of the service area without regard to an individual's ability to pay or health insurance status.

Federally-funded health centers care for you, even if you have no health insurance. You pay what you can afford, based on your income. Health centers provide:
  • checkups when you're well
  • treatment when you're sick
  • complete care when you're pregnant
  • immunizations and checkups for your children
  • dental care and prescription drugs for your family
  • mental health and substance abuse care if you need it
Participating Consolidated Health Care Centers are located in most cities and many rural areas.

General Eligibility Requirements
All people are eligible to receive health care services at a federally funding health center. Health centers funded through the Consolidated Health Centers Program are required to provide services to all individuals regardless of ability to pay or health insurance status. Federally funded health centers also must provide a schedule of discounts corresponding to established charges for individuals with incomes at or below 200% of the Federal Poverty Guidelines.
